D’Viajeros Health Declaration – Easy Step-by-Step Guide

Do I need to fill out the health declaration?

If you’re traveling to Cuba in 2025, yes, you’ll need to complete an online health declaration before entry.

To make this quick and easy for you, we have created a quick guide to filling out the health declaration for Cuba.

Every traveler must complete a health declaration, meaning one form per person is required. However, keep in mind that the form can only be filled out 48 hours before your arrival in Cuba. If you are trying to access it earlier and it’s not working, don’t worry—it will become available closer to your departure date.

Step 1 – Switch the page to English

The site is basically set up in Spanish, and so are the instructions. If you don't speak Spanish, it may help to switch the site to English.

To do this, click on “Lenguaje” in the top right corner and select “English” (English) as your desired language.

Optionally, Google also offers you the possibility to automatically translate the entire page into another language – perhaps it is easiest for you if everything is in English straight away?

Then click on “Create form” to start the declaration.

Step 2 – Personal information

Now it’s time to fill in your personal details.

Tip: Required fields are marked with (*), so you don’t have to fill in everything. Below, you’ll find an image that shows you how to complete each section.

The fields "Middle Name" and "Middle Last Name" are available for entering the middle name and the middle last name. Of course, these can also be left blank.

Please fill out everything correctly! The fields will be outlined in green if filled out correctly.

Step 3 – Flight information

In the second part of the health declaration you must provide your flight details.

You can find this information on the airline's online booking portal. Log in to the airline's website with your booking number to find all the information you need.

Here are the names of the 3 main airports in Cuba:

• Havana: Aeropuerto Internacional José Martí

• Varadero: Juan Gualberto Gomez International Airport

• Holguin: Holguin Airport

Note: It is not mandatory to provide your seat number. It is usually only assigned at check-in.

Step 4 – Health information

In this step, you must indicate the province you plan to travel to. You must also indicate the municipality, the first address and the type of accommodation.

A Casa Particular is referred to as a “private rental.” Cuba Buddy travelers will find the address of the first accommodation in their travel documents.

As of April 6, 2022, a PCR test (or antigen test) and proof of vaccination are no longer required to enter Cuba.

Step 5 – Customs declaration

The customs section of the health declaration requires a bit more attention since you need to specify exactly what you're bringing with you.

First, indicate whether you have unaccompanied baggage and if children under 10 years old are traveling with you.

Next, you must declare how much cash you’re carrying if it exceeds 5,000 USD or the equivalent in another currency. Be sure to select the correct currency and enter the exact amount.

To add this information, click the plus symbol to insert a new row permanently. You’ll see a black border around the newly added section, as shown in the image below.

Then, declare any other items you are bringing with you. Follow the instructions on the website and choose the correct item from the list.

Finally, click yes or no to confirm whether you need to declare anything to customs.

Step 6 – Complete health declaration

To finally submit the health declaration for Cuba, you must confirm that all the information provided is correct.

As soon as you click on “Next”, the form will be sent immediately.

Step 7 – Download confirmation

After submitting the health declaration, it is recommended that you download and save a copy, just to be on the safe side.

Make sure to also save your private code, as this will allow you to edit the details of your health declaration for Cuba later if needed.
